I think in this case the main retainer ring which holds the lens assembly to the focussing helicoid has become loose. Unfortunately on this camera this ring can only be accessed after removing the complete front assembly. This is not very difficult but not a beginner's job.
After removing the front assembly, remove the plastic light guide and unscrew the rear lens cell. Beneath it there is a small chrome ring which should be tightened.
